NHS Braces: Who Qualifies?

NHS orthodontic treatment is free for children and teenagers under 18. However, not everyone qualifies. Eligibility is determined by the IOTN (Index of Orthodontic Treatment Need), a scoring system that assesses how much orthodontic treatment a patient actually needs on clinical and aesthetic grounds.

The scale runs from 1 (no need) to 5 (great need). NHS treatment is generally offered to children scoring grade 3, 4, or 5. In practice, due to funding pressures, many areas only routinely offer treatment to grades 4 and 5.

Children with mild crowding or minor spacing issues (typically grade 1 or 2) will not qualify for NHS treatment. This comes as a surprise to many parents who expected treatment to be available for all cosmetic concerns.

The referral process: your child's dentist assesses whether a referral is warranted, then refers to an NHS orthodontist. The orthodontist carries out the formal IOTN assessment and decides whether treatment is funded.

NHS Waiting Times

Even for children who do qualify, the NHS orthodontic waiting list typically runs 12-24 months. In some parts of England and Wales, waits exceed 2 years.

This has practical consequences. A child referred at age 11 may not start treatment until age 13, then spend another 18-24 months in braces, completing treatment at 15-16. Children who are referred later, or who wait longer, may approach the age 18 cutoff before finishing.

If your child is close to 18 and has not yet started NHS treatment, it is worth checking the status of any referral. NHS treatment must begin before a patient turns 18 to be funded.

Prices at the upper end of these ranges are typical in London. Outside London and in smaller cities, prices tend to be £500-£1,500 lower. These figures include all appointments and adjustments but may not include the initial records fee (typically £100-£300) or retainers after treatment.

NHS vs. Private: Should You Go Private?

If your child qualifies for NHS treatment and the waiting time is manageable, NHS braces are excellent value. The clinical standard of care is the same as private. The difference is choice: NHS typically means fixed metal braces, whereas private gives you access to ceramic, Invisalign, and lingual options.

Families often go private for three reasons: their child does not qualify for NHS treatment, the waiting list is too long and the child is approaching 18, or they want a specific type of brace (typically Invisalign or ceramic).

Adults in the UK have no NHS orthodontic provision unless there is a clinical health need (not just cosmetic). All adult orthodontic treatment is self-funded.
